2012 Member Directory Information
A new membership directory will be assembled over the summer and will available at our first meeting in the fall. In order to have the correct information for each member listed, it is essential that you fill out these forms in their entirety and return it to us by email or mail no later than June 15, 2012.
Please make sure that you include your email address. Costs of printing and mailing have become prohibitive and we will be turning to email to deliver important and time sensitive information to you in a speedy and cost effective manner. National and Regional information is email only.
This year’s bulletin will still be delivered by mail, but that may have to change in the near future.
Please note, you can use open office or google documents to open and edit word documents. For PDF documents, you can print and scan them or print and then mail them in.
